Cancelation Fee:

• If you cancel or reschedule your appointment less than 4 hours prior to the scheduled appointment time, you will be charged 50% of the cost of the service. When you rebook, a 50% deposit is required for the new appointment.    

No Show Fee: 

• If you do not show up on time for your appointment, you will be charged the full amount (100%) of the scheduled service and advanced payment is required prior to booking your next appointment.

Late Fee:

• If you are not at your appointment on time, you are late.  However, there is a 7 minute grace period.  In this case, your treatment time will be shortened and charged the full amount (100%) of the scheduled service. For example: Late 7 minutes for a 60-minute treatment. The treatment (if applicable) will be shortened to a 45-minute service. 

Pre-Paying & Deposit:

• Paying before your appointment secures your appointment.  

The Right To Refuse Service:

• Inappropriate behavior will not be tolerated and you will be asked to leave and seek services elsewhere.   

Redeem Your Gift Certificate:

• To redeem a gift certificate, bring it with you to your appointment.

I Forgot My Gift Certificate:

• Payment for services will be due at time of service.  You can use your gift certificate at your next appointment.

Only Santia Day Spa gift certificates are accepted

Client Form:

• Tells me about you and how you view your skin health.  This helps me to offer a customized facial that best fits your needs, goals and concerns.

Client History Form:

• Provides information about your health.  This helps me to determine if I should refrain from using certain products or modalities during the service. 

Additional Consent Forms:

• Will be utilized for specific services.

Instructions:

Forms will be sent to the email address you provided at the time of booking the week of your appointment.  Please print, fill them out and bring them with you to the appointment.
